Description
The Community Rehabilitation Service focuses on providing rehabilitation support to patients within their own communities, enabling them to achieve their goals and regain lost skills.
Our team empowers patients to increase their levels of independence, wellbeing, and makes lasting changes for their future. We provide support and guidance to help them make informed decisions about their treatment.
Main Duties of the Job
To provide high-quality patient-centered care and specialist rehabilitation to adults referred to the Rehabilitation Service.
Participate in the development of the Rehabilitation Service in the community and within the Community hospital bedded environments.
Provide specialist advice regarding the rehabilitation management of patients to families, carers, and other members of the Multi-Disciplinary Team (MDT).
Support and guide Band 5 Therapists, Apprentices, Rehabilitation Assistants, and Students.
Responsibility for a caseload of complex patients within both the community and the Community hospital bedded environments.
Act as Clinical Supervisor and undertake Managerial Supervision, including Appraisal for Band 5 Physiotherapists and Rehabilitation Assistants.
Assume Team Lead responsibilities in the absence of the Rehabilitation Manager, supporting staff and attending meetings delegated by the manager.
