We have an exciting opportunity to onboard a new member to our Cambridge City North Nursing team, working with us, you will be fully supported to gain new skills and experience needed to deliver high quality care to patients in their own home or residential setting in and around Cambridge City Centre.

We provide a service 7 days a week, working closely with other community services and Primary Care Networks to provide high standard holistic care to the population of Cambridgeshire

You will have the opportunity to develop yourself, with extensive learning and development opportunities.


As an Integrated Care worker you will work closely with the nursing team to follow a plan of care in deliver high quality, best practice care to adults with a range of complex health needs within their own homes.

We are looking for someone with:
Experience of working in the community.

Experience of working within health or social care.

Experience of caring for and supporting adults and older people

Demonstrate a commitment to high quality care & service provision.

Good communication skills.

The ability to work flexibly and as part of a team, as well as individually.

To undertake patients prescribed treatment/care plans, i.e., venepuncture, insulin administration and wound care, and report findings to a qualified professional in line with competency framework.

Communicate with patients/carers by exchanging factual information, reassurance, tact and empathy.

Establish and maintain appropriate working relationships with colleagues, patients, carers and other health care professionals.


Work under guidance on specified tasks, taking direction from registered and non-registered staff as appropriate, with regular clinical support and supervision by a qualified practitioner.


Promote patient independence and self-management Provide and maintain a safe, comfortable and dignified environment for all patients/clients and carers To practice the principles and philosophy of individualised patient care in accordance with Trust Policy, procedures and regulatory body's guidelines.

To maintain appropriate and timely documentation of each patient contact (ensuring these are recorded on SystmOne within 24hours).

Plan and prioritise own visits to delegated patients, (and ensure these are documented on SystmOne).
