Role Provides prompt, efficient coordination and administration support to a multi-disciplinary team (MDT) within the Health and Social care setting.
Works with relevant MDT professionals to support implementation of Care provision and tracks progress of Care.
To ensure a prompt response to crisis referrals and co-ordinate interventions by team members or ensure referrals are made promptly to other services to prevent unnecessary hospital admission.
To maintain the information systems used by the team in order to collate and co-ordinate performance Data and produce reports as required for the Team and relevant managers.

To gather further information for team members regarding patients/customers
from referrers, agencies & teams as required.
To act as co-ordinator to ensure actions by members of the MDT have been completed in a timely manner.

packages of care at short notice to prevent unnecessary admission to
hospital. To provide cross cover to other teams as required to cover leave, sickness and training.

To develop an understanding of community resources/services available
within the community to meet client needs e.g. to support initial contacts/ sign posting of information or process referrals.
To ensure the information is produced in an appropriate format and made
available as required to team members and members of the public.
To ensure the community resources/service information list is kept updated as necessary.
To use IT systems, inputting referrals, assessment and care
requirements and allocating cases on the Social Care system.
To collate, input client related (or other required) data and produce relevant data for statistical analysis and to produce reports using IT packages including Excel and Access.

To sign post referrals to team members and other services as appropriate.
Monitors, tracks and reports on progress within the patient pathway.
Highlights any issues or delays to relevant MDT professional for appropriate
action. To process and administer referrals made to proactive care according

team members when required.
To provide a client centred approach to ensure all clients' and carers' views and opinions are considered and all people who make an enquiry are given respect and dignity.

To work with and provide administrative support to the MDT members to
produce detailed care plans. For use in the ongoing provision of care.
