JOB PURPOSE The post holder will play a key role in supporting the Personal Health Commissioning Service.

Working within the remit of the National Frameworks for Continuing Healthcare, the post holder will deliver an effective and competent level of service and consistently deliver a client-focused service.

NHS Dorset currently offers a hybrid model of working with a percentage of contracted hours being on site and the remainder being home based.

The post holder should hold a full UK driving licence and have access to a vehicle to be able to fulfil the requirements of this post.


POST SPECIFIC, TASKS AND OBJECTIVES Undertaking Retrospective Assessments to determine if an individuals support requirements indicate a Primary Health Care Need (within the definition of such in the National Framework for Continuing Health Care) and therefore eligibility for Continuing Health Care Funding.

Supporting Decision Making functions by forming part of a team rota and input as required. To ensure all recording and assessment reports are presented in a highly professional manner. To work diligently to secure outcome decisions as quickly as possible to provide resolution for individuals.

Communicating confidential and contentious information as relevant in a skilled manner showing sensitivity and empathy with all stakeholders.

Supporting the Team Lead to ensure that all processes implemented are compliant with the NHS Continuing Healthcare National Framework Assisting the Team Lead with resolution of complaints, appeals and disputes.

Raising issues of concern through the Senior Management Team where necessary Contributing to performance improvement within the team of Senior Practitioners, Practitioners and Associate Practitioners ensuring that services are delivered to a high standard.

Working with members of the team to investigate the causes of any variance from target/plan and proactively contribute to the implementation of solutions.


Actively supporting the Personal Health Commissioning team in working towards goals and objectives Maintaining professional registration Having and maintaining an in-depth and up to date knowledge of the National Framework for NHS Continuing Healthcare and NHS-funded Nursing Care and Children and Young Peoples Continuing Care as well as associated policies, legislation and legal judgements that impact on the delivery of the service.

Delivering against objectives, achieving quality outcomes, prioritizing own workload, and working to tight deadlines Fostering transparency and a person-centred approach.

Understand professional and personal role in relation to safeguarding processes and procedures in NHS Dorset and Local Authority Partners Working with health and care colleagues to ensure a quality service is always delivered.

Understand the importance of efficiency, effectiveness and best value in all working and commissioning practices.

The post holder will be responsible for ensuring their mandatory training is up to date.

Personal Health Commissioning is a challenging area of work where staff face stressful and emotional situations, and the post holder will need to manage these situations in a supportive and compassionate manner both with individuals and their representatives and the staff working in the service.
