The North Bristol Trust Transfer of Care Hub are seeking an individual who is dynamic, responsive, flexible and self-motivated, with experience of planning and delivering service transformation initiatives.

As an Improvement Lead you will work to support the Discharge-focused Senior Team within the Transfer of Care Hub and in the wider hospital to support service improvement.

You will directly support the delivery of our Hub vision 'to ensure a hospital bed is available for everyone when they need it by empowering and enabling people to leave hospital at the earliest opportunity'.


You will work with a range of clinical and non-clinical teams to establish and deliver key initiatives both within the Hub and across the Trust to improve patient care and patient /staff experiences relating to flow through and discharge from hospital.

The Transfer of Care Hub is a team of teams based at Southmead hospital which includes partners from the hospital, Community Health Care and Social Care teams from across our three Local Authorities and Voluntary Sector Organisations to ensure patients are discharged from hospital to the right place at the right time.


The post holder will lead the delivery of transformation programmes for the NBT Transfer of Care Hub within the overall Transformation programme.


The core elements of the role include:
Project and programme management

Service improvement and redesign skills

Facilitation and communication expertise

Building staff capability for improvement through training, coaching, and mentoring.

The exact nature of day-day tasks depends on the agreed projects. However, all projects will involve updating project documentation, facilitating conversations and workshops, and the use of service improvement tools and methodologies
