Description

Role Summary
To co-ordinate services, using CM2000, to ensure provision of high-quality, person-centered care in the community.

To work in partnership with the Local Authority and other external relevant parties, such as Health and Social Care Partnerships (HSCPs) to further expand the business through procurement of work.

To maintain accurate records on Company shared systems and software to ensure compliance with the QMS and regulatory requirements.

To promote and safeguard the welfare of Service Users and staff in line with the Adult, Support and Protection (Scotland) Act, 2007.

To report directly to and provide support to the management as requested, including participation in team meetings.


Objectives, Main Duties & Responsibilities

  • To be willing to achieve an SVQ Level 3 and PDA in Supervision or equivalent to ensure compliance with the Scottish Social Services Council (SSSC)and Care Inspectorate Conditions of Registration.
  • To ensure that care services are delivered to Service Users in accordance with the agreed care package.
  • To be Service User focused and deliver an outstanding Service User experience.
  • To support, maintain and develop relationships with new and existing Service Users.
  • To carry out regular care reviews and update care plans as required.
  • To facilitate the change in Service Users' care needs and to ensure appropriate recording and management of the changes with respect to Care Worker workload and rotas.
  • Assist with the creation of care review schedules for existing Service Users.
  • To develop close professional working relationships with Care Managers and allied health professionals ensuring regular meetings are held to maintain relationships and identify changes in care needs.
  • To regularly meet with and manage Care Workers in accordance with the QMS ensuring a close working relationship.
  • To cover visits in the event of Care Worker cancellation when all alternatives are exhausted.
  • To ensure accurate recording and documenting of any information received from Service Users or Care Workers on the appropriate Company IT systems.
  • Ensure all enquiries, compliments, and complaints are documented and passed to the appropriate person(s) in a timely manner so they can be dealt with immediately and appropriately in accordance with the QMS.
  • To take telephone calls promptly and effectively, imparting all relevant information to appropriate operations staff.
  • To be part of a professional and competent rotational on call service covering evenings, weekends, and public holidays. This may vary in frequency to meet the demands of the service.
  • To be willing to work in other geographical areas in line with the Company's requirements.
  • To work in a manner which complies with the Health and Social Care Standards set out by the Care Inspectorateand SSSC.
  • To promote a high level of health and safety awareness and commitment, and to be aware of the provisions as detailed in the Company Health & Safety Policy.

Salary:
£22,000.00-£28,000.00 per year
