We currently have a great opportunity for a
Care Coordinator to join our Care Plus team on a permanent basis.
This role will be based across our 4 Retirement Living Plus schemes in Essington, Perton, Cannock and Penkridge in South Staffordshire.
We're looking for a dedicated professional to bring our values to life and ensure our customers receive outstanding services.The Role
As our Care Coordinator, you will manage, support and monitor the delivery of care and support services across our four schemes.
You will monitor all care and support plans and other records are maintained and reviewed in line with company procedures and you will liaise with service user families, carers and other professionals to ensure service user needs are being met.
Skills and Experience required:
- NVQ Level 3 in Care or equivalent (desirable)
- Computer literate with a good working knowledge of Microsoft Office
- Excellent written and verbal communication skills
- Previous experience in providing a care service for people
- Supervisory experience and rostering experience (of up to 1 year)
- Full UK Driving Licence and access to your own vehicle for business use
