Duties:

• Assess the needs of clients and develop personalized care plans
• Provide guidance and support to senior carers, carers and other healthcare professionals
• Conduct regular assessments and reassessments to evaluate client progress
• Collaborate with healthcare providers, families, and clients to ensure effective communication and continuity of care
• Maintain accurate and uptodate client records
• Provide supervision to carers in the service

Requirements:

• Proven experience in a senior role or a similar role
• Strong knowledge of working in the care field
• Excellent communication and interpersonal skills
• Ability to assess client needs and develop effective care plans
• Proficient in using electronic health record systems
• Compassionate, patientcentered approach to care

Salary:
£25,000.00-£26,000.00 per year

Expected hours: 40 per week
