The post holder will provide dedicated support to the CHC Heads of Service which included, The Head of CHC and Head of CHC Operations.


The role is responsible for the ongoing development, delivery and review of key All Age CHC Improvement work programmes as directed by head of service.

The role will provide a high-quality administrative support within an agreed CHC Improvement work portfolio, including reviewing, developing and managing the Transformation plans, monitoring progress and risk and issue management / reporting

  • Liaising with the appropriate leads across the programmes, to support improvement through best practice and redesign.
  • Supporting the workstreams/board meetings and the Transformation manager.
  • Enabling and promoting the effective use of data, information, knowledge, and technology to improve and inform Transformations and service initiatives within the work portfolio. This will include including providing support to the Transformation lead to ensure that Transformation outputs are cost effective, delivered on time and to a highquality standard
  • Managing all Transformation returns and reporting, including progress reporting to the Groups and Programme Board, and providing appropriate Transformation delivery updates to service leads and other stakeholders when required.
  • Developing, implementing and maintaining a content management system to ensure information is properly managed and is shared across the team as appropriate with oversight and management of all aspects of the Transformations
  • Supporting Heads of service by devising and delivering required training in relevant Transformation areas.

Following an exciting re-structure in the Continuing Healthcare Team we are now looking to appoint a newly created position of a Occupational Therapist.


  • Communicate, establish, and maintain constructive relationships with a broad spectrum of key stakeholders, gaining their cooperation to support successful delivery of programmes of work within the portfolio in agreement with the Heads of CHC and Transformation Manager.
  • Participate in relevant internal and external working groups/Transformations to provide information/analytical advice and expertise to the Transformations.
  • Create and present information, including complex issues through the development of briefings and reports to a wide range of internal and external stakeholders
  • Prepare and produce concise yet insightful communications for dissemination to senior stakeholders and a broad range of stakeholders as required relating to Transformation
  • Develop and assist in the maintenance of Transformation documentation to monitor progress and resources
  • Work directly with the programme to support the programme meetings and outcomes and support appropriate management and recording of information
