**PLEASE NOTE - this role will be a split site role & the sites are: Alexandra Avenue Health and Social Care Centre & Honeypot Lane Centre for Health

  • You will work in collaboration with other health care and service providers to ensure seamless delivery of services for clients and communities

  • The post holder is responsible for the day to day running of the District Nursing team to high standards within the locality. The post holder leads the team in monitoring and improving standards of care and assists the overall development of the adult nursing service, delivering a cost effective service within the boundaries of available resources while ensuring patient safety and ensuring clinical standards are met.
  • The post holder will also provide managerial and clinical leadership to community nursing & Allied health professional staff, and act as an effective role model for multidisciplinary staff members and other team members, providing evidencebased clinical advice and support to all staff within the locality.
