About the role.
This is a full-time post (37hrs) fixed term for 2 years

This role is based within a team that carries out reviews for those in receipt of social care services.

You will work with a wide range of people and will be required to undertake reviews of individuals support and put in place any necessary revisions to ensure the plans remain relevant, proportionate, appropriate and are effective in delivering any required outcomes.

You will be required to undertake assessments where appropriate with adults and/or their carers.

You will identify individualised, outcome led personal support, based on structured, individual assessments in order to minimise or eliminate an individual's dependency on long term social care.
