Description

We currently have an exciting opportunity within the Medicines Management Team for a full-time Care Co-ordinator (Patients on Specific Medication). This would suit an organised, enthusiastic and methodical administrator. This is a new role attached to Trowbridge Primary Care Network, working across both practices.
Main duties of the job


The role will involve working closely with the Clinical Pharmacists and the Medicines Management Team - to identify and organise blood tests for patients who are using specific medication.

Trowbridge Primary Care Network covers the Trowbridge area and a population of almost 50,000. There is a strong team spirit throughout the organisation. Both practices enjoy working together to produce the most efficient services for our patients within our town.
