We have an exciting opportunity for a Community Support Worker for 30 hours a week in the Physical Disabilities team.

You will work as a member of the team, alongside Social Workers, providing a frontline service for customers, services providers and other agencies.

We are a small, friendly and supportive team.

You will be assessing persons needs under the Care Act 2014.
