Memory Assessment Practitioner - Band 6Newham Diagnostic Memory Clinic.

Are you passionate and committed to supporting people with Dementia and Memory problems?


We are looking for motivated and enthusiastic Registered Mental Health Professional with specialist interest and experience in assessing and caring for people with Dementia and memory problems to join our Diagnostic Memory Clinic.

The post is full time.


As a well-established Team with motivated, proactive and supportive team members, we care for service users with complex needs, including severe and enduring mental health needs, dementia and cognitive impairment, requiring us to provide skilled assessment and interventions through the integrated Health and Social Care model of care.


We are currently undergoing an exciting time of transformation, working in partnership with our service users, their carers, Local Authority, voluntary organisations and other stakeholders in a drive to continue our goal of providing inclusive and high quality care while developing our service beyond the traditional scope of health and social care.


The Team promotes opportunities for professional development through regular supervision, access to trainings, including managerial and leadership and quality improvement (QI) projects.


  • To work as specialist practitioner in the Diagnostic Memory Assessment Service within the CHMT for Older People of the Newham Locality.
  • To provide specialist assessment, care and support to people presenting with cognitive impairment, dementia and comorbid health care needs.
  • Actively engage people with Dementia and Cognitive impairment and their carers in all stages on intervention, ensuring that their views are acknowledged and taken into account.
  • Assess and present individual service user cases in MDT meetings to review their care needs.
  • To laise with the GPs, Local Authority and other statutory organisations and voluntary agencies to promote partnership working.
To develop and facilitate specialist groups within the Team to support those diagnosed with Dementia and their families
