Job Summary:
To act as the first point of contact for Patient Safety Team queries (both internal and external)

To act as a point of contact for the Governance and Risk team To be a point of contact for all staff providing support, advice, troubleshooting and resolution of issues relating to the Risk Management System Contribute to the overall governance and risk improvement agenda, alongside the Delivering Excellence Framework Provide senior administrative support to the Governance and Risk team and Patient Safety Team, supporting compliance administration as required Support training staff at all levels in the use of all the Risk Management System modules Key Responsibilities:

Manage and coordinate the Patient Safety Team (PST) mailbox, ensuring all queries are responded to within a timely manner.

Manage and coordinate the External Incident mailboxes, ensuring all queries are responded to in a timely manner. This includes the co-ordination of the investigation process to ensure the timely review and dissemination of learning from events.

Manage the process and administration for the incident triage and weekly Incident Review Panel (IRP) and PHU/ SCAS incident review meetings.


This will include, but is not limited to:

  • Reviewing all reported incidents graded as moderate harm and above and all those that are referred to the Patient Safety Team
  • Co-ordinating the invite list for the above meetings
  • Ensuring agendas, checklists and paperwork associated with the meeting is collated and distributed in accordance with the Terms of Reference
  • Taking notes, actions and responding to queries as appropriate
  • Updating the Trust Risk Management System to ensure accurate and auditable information is held on record
  • Drafting a weekly report to the Trust Leadership Team with the outcome of the IRP Collate incident reports received after investigation for sign off, amend as applicable and save onto the Trust Risk Management System for final review Liaise with Integrated Care Board (ICB) colleagues on a regular basis on behalf of senior colleagues Attend and provide administrative support to the PST in relation to attendance at Patient Safety Incident Response Framework (PSIRF) meetings Support and maintain the learning from deaths tracker to include Learning Disability deaths information sent by the Medical Examining Officers and requesting and tracking notes in order that Structured Judgement Reviews (SJR) can be completed Update and review investigation templates as necessary to reflect any National or local changes implemented Provision of, and analysis of information as requested by the Governance and Risk Department, Patient Safety Team, Divisions, Care Groups or Trust Executives Work independently (with supervision when required) to plan and prioritise tasks and activities which may be required at short notice due to variable workload / interruptions Provide ongoing advice and support for all staff in relation to risk and patient safety processes ensuring that any issues arising are resolved promptly Carry out regular audits of incident data quality to ensure that any issues are identified promptly and that the necessary action is taken to improve recording Ensure that information is provided, as required to external agencies (e.g., Care Quality Commission, National Patient Safety Agency, and ICB) in a timely manner Produce reports and provide data and information by extracting information from all Risk Management System modules and data held locally for internal and external reporting purposes. This will involve analysis of both basic and complex data and provision of regular statistics to inform management reports for senior managers and Trust meetings.


These reports will be required as evidence for internal and external scrutiny processes Liaise with staff at all levels in the organisation including Senior Managers and colleagues in other agencies, such as the ICB, ICS and NHS England as appropriate.

Provide support to the Risk and Systems Manager and the Deputy Director of Governance and Risk to ensure all the responsibilities of the Governance and Risk team are fully covered during leave periods, including general administration tasks Contact with patients is incidental although, due to the nature of the department responsibilities, exposure to sensitive information and emotional circumstances occurs Key relationships:
Communicating complex, sensitive information with a range of staff internally and externally across the organisation/organisations; liaising with external organisations, to include the following: Deputy Director of Governance and Risk Deputy Director of Nursing for Quality and Safety Governance and Risk Team Patient Safety Team Divisional Directors, Directors of Nursing and Operational Directors Divisional and Care Group Governance leads (medical and non-medical) Clinical and corporate staff NHSE/I
