Description

We are looking for an organised, motivated and enthusiastic Outreach Support Worker to join our Empowerhouse Outreach Team.

This is an exciting opportunity to join our Team, providng on
- call outreach support to young people between the ages of , accross all 7 of our provisions.

The On-Call Support worker will liaise directly with each home to support them with emergency appointments, meetings or any other outreach requirements for that day for the young people.

You will have the opportunity to work with many young people with differing needs and risks, building a strong rapport with the young people, the team, social workers and third party agencies we work with.

You will be expected to;

Keep track of your Daily/Weekly Schedule
Complete your outreach authorisation forms
Write up Keywork sessions folling all outreach delivered
Be available on the days you are oncall for last minute outreach needs
Keep track of any additional expenses/receipts
Have an Enhaced, Clear DBS
Have your own Vehicle, with a valid licence, insurance and MOT
2 years experience within the same or similar role
Able to travel to Wembley, Hillingdon, Richmond area's within London

The on-call rate is £40.00 per day to be on standby, and £12.00 per hour for the outreach delivered. (The hourly rate is calculated from the moment you leave your current location, until you have completed your outreach support)

The on call weekly rate is £225.00 per week to be on standby, and £12.00 for all outreach delivered.

You will travel utilising your own vehicle, and will be paid 0.45p per mile
