One _Norwich Practices is looking to recruit an enthusiastic and dynamic care coordinator to work within the INTERACT service.

This role is part of an exciting new project working in partnership with district and borough councils and the voluntary sector to support people whose health and wellbeing is negatively effected by the housing or housing environment.

This project is taking a new approach to supporting people through the innovative use of local data and intelligence.

In partnership with local councils, people living in under or over-occupied housing with additional health needs will be contact by the care coordinator to offer support such as social prescribing, home improvements and adaptations and practical support with moving house.

The project team will be made up of two social prescribers and a volunteer coordinator, as well as the care coordinator and will work closely with housing improvement teams in local councils.
