Job overview:

Do you have a passion for providing a quality, innovative service to forensic community clients with learning disability and or autism spectrum conditions? Are you keen to work alongside other criminal justice agencies as part of the centre for Mental Health and Justice to provide specialist care coordination and consultation to this client group both in and out of county? Do you value the principle o least restrictive risk managed care as close to home as possible?

The specialist part of the forensic team based at the Bodmin Police Hub is developing and expanding its service to this client group and you will be working as care coordinator alongside an established psychologically informed team

Knowledge and experience of working with individuals who have learning disability or autism spectrum condition needs is essential


Main duties of the job:


To act as a care coordinator to an identified client group who have community forensic needs and have a learning disability and or autism spectrum condition.


To liaise with and inreach to providers and units as part of the secure care services both in and out of county.


To develop robust care plans and effective risk management strategies to ensure high standards of safe, creative and individualised clinical care is provided.

To provide specialist advice and consultation as part of the wider team
