Description

The post holder will support the Coventry and Warwickshire Prevention Senior Responsible Officers to ensure effective delivery of NHS Long Term Plan (LTP) Prevention priorities across the C&W system working within NHS Coventry and Warwickshire Integrated Care Board (ICB) and linking with wider healthcare provider and local authority partners.


This is a critical role central to our ICB four aims:
Improve outcomes in population health and healthcare

Tackle inequalities in outcomes, experience and access

Enhance productivity and value for money

Help the NHS support broader social and economic developments


The NHS LTP Prevention priorities include programmes to strengthen NHS Prevention activity in the following areas; tobacco, obesity, alcohol and tuberculosis.

A significant focus of the role will be supporting effective delivery of the NHS Smoking Cessation pathways across multiple trusts and community pathways.


Your role will support NHS ambitions around tobacco control:
All people admitted to hospital who smoke will be offered NHS funded tobacco treatment services

The model will be adapted for expectant mothers, and their partners with a new smoke free pregnancy pathway

A universal smoking cessation offer will be available as part of specialist mental health services for long term users of specialist mental health and learning disability services

Lead coordination and delivery of programmes and projects aligned to the NHS Long Term Plan Prevention priorities and funding streams

Act as project support to the LTP Prevention Steering Group and its subgroups

Support embedding prevention across NHS pathways and align work to the wider system strategy and Health Inequalities Strategy

Work with the wider Population Health Management and Health Inequalities tea

Act as a system champion for prevention embedding prevention and inequalities into CWICB work

Take a lead role in C&W system delivery of NHS Smoking Cessation pathway development, managing specific elements of the programme, developing project plans, reporting progress and undertaking risk and issue management

Use strong analytical and judgement skills to translate national requirements into local system solutions

Use judgement and negotiation skills to reach consensus with partners on system solutions taking into account complex situations, diverse options and varying timescales

Liaise with key partners across C&W on joint project matters

Lead development of system level activity to support prevention activity, which may include:
workforce training and development

development and refinement of clinical pathways

communication and engagement plans

shaping IT and data sharing solutions

reporting to the Population Health, Inequalities and Prevention Group and NHSE/I

shaping quality improvement activity across pathways

This is an exciting opportunity to join our Population Health and Inequalities team within the CWICB.

This role has been created to support our ongoing prevention activity and will work alongside our Health Inequalities Programme Managers and wider system partners to progress this agenda.

While CWICB is the host organisation for the post, the role spans our health and care system and will involve working across a diverse range of partner organisations.

Communication

Write and present reports and presentations including data from multiple sources to a variety of audiences including senior management and clinicians

Maintain constructive relationships with a broad range of internal and external stakeholders

Have excellent communication skills and the ability to persuade, motivate and negotiate across multiple system partners at all levels of seniority

Lead project delivery with excellent external and internal communication

Interpret and implement local response to emerging national prevention priorities and funding streams

Lead day to day system communication with NHSE/I supporting integration of complex programme requirements into local system solutions

Lead project reporting and communication, including highly complex and sensitive data and intelligence from a range of organisational partners and presenting a system view on programme progress and system level data reporting

Liaise with wider ICB workforce to share best practice in prevention

Planning and Organisational Skills

Lead and manage projects to further NHS Prevention aims using project and relationship management skills

Work across varied teams at system, place and provider level to ensure effective delivery

Act as budget holder for system prevention funding coming into the ICB, providing oversight and monitoring

Develop and implement data collection systems that will provide accurate and timely data and reporting

Comply with aspects of data protection and data access

Plan and prioritise own workload working on own initiative
