Description

Key duties To undertake chronic disease management (i.e., asthma, diabetes, hypertension, coronary heart disease), incorporating monitoring progress, response to treatment, reducing unnecessary hospital admissions and promoting the preventative aspects of clinical practice.

Provide comprehensive family planning service including advice on emergency contraception. Initiate, perform and interpret ECGs, acting on results accordingly. Initiate laboratory requests and collect specimens as required.

Interpret results and act accordingly. Manage commonly occurring minor illnesses/injuries and refer to other health professionals as appropriate. To recognise abnormalities and screen patients for disease risk factors and early signs of illness. Support other services provided within the general practice setting within agreed parameters.


Audit & Research Essential Ensure that effective communications systems are set up, which include referral protocols, guidelines and policies for the management of patients in conjunction with other members of the PCN.

Continuously evaluate and review activity and the effectiveness of care through audit.

Participate in relevant research programmes and explore opportunities to undertake research in order to enhance the body of knowledge in a specialized field as appropriate.

Disseminate research findings appropriately to other members of the PCN as well as the wider healthcare field.


Play an active role in the Clinical Governance agenda at practice level and act as a resource to other practice members.

Provide ongoing assessment of local health needs via profiling. Management Essential Ensure the maintenance of accurate records in line with practice policy and UK guidelines. Compile relevant statistics to meet the requirements of the PCN and Health Authority.

Participate, together with other members of the PCN in service reviews. Initiate and maintain appropriate Clinical Supervision for the clinical team within the practice. Maintain own professional development in line with Post-Registration Education and Practice (PREP). Explore opportunities to network with other similar post-holders, to share ideas and provide professional support.

Establish an effective stock control system. Attend clinical, managerial, PCN and other meetings as required.

Professional Development Essential To continue professional development through a variety of strategies including attending study days, clinical placements, lectures and appropriate courses.

Act as mentor to other clinicians where appropriate.

Act as tutor to student and conversion course clinicians as appropriate.

The postholder will participate in any training programme implemented by the PCN as part of this employment, such training to include:

Participation in an annual individual performance review, including taking responsibility for maintaining a record of own personal and/or professional development, Taking responsibility for own development, learning and performance and demonstrating skills and activities to others who are undertaking similar work, Training may need to be undertaken outside of normal practice hours, and off site.
