Stockport NHS Foundation Trust is looking for an enthusiastic, caring, and experienced Assistant Practitioner to be part of their Adult Community Therapy Team.

You will be an enthusiastic practitioner who will work both independently and collaboratively, visiting patients in their own homes and clinics.

You will work closely with Occupational Therapists and Physiotherapists to deliver rehabilitation programmes for patients who have a physical disability or a long-term condition, as well as patients recently discharged from the acute or D2A settings.


  • You will work in the community as an autonomous practitioner visiting patients in their own home, providing rehabilitative domiciliary programmes and intervention, under the guidance of qualified community therapy staff.
  • You will progress patients in line with your own competencies and under the supervision of a qualified therapist.
  • You will assess for and provide walking aids in patient homes and clinic settings.
  • You will also contribute to the supervision of junior staff and students
  • You will follow all Trust standards, policies, procedures and competencies appropriate to the band/position.
  • You will maintain accurate and uptodate electronic data and records using the approved system and following national and local standards.

The successful applicant will carry out treatment programmes directed by the Physiotherapists and Occupational Therapists, with regular reviews of patient goals, and progression of rehabilitation programmes.

Alongside direct patient care, the post holder will support the team with admin, service development and housekeeping activities.

If successful this vacancy will require an enhanced DBS check at a cost of £41.90. This cost will be deducted from your first Trust salary


In addition to this it will be a condition of your employment that you will join the DBS update service and pay the annual subscription fee of £13.
