Description

The post holder will be part of an exciting new team delivering a Tobacco Dependence Treatment Service to patients who are identified as smokers when admitted into Hospital.


They will work closely with the Tobacco Dependence Treatment Service Lead in supporting the roll out of a Trust wide Smoke Free service.

Smoking Cessation Practitioners will be expected to engage with patients in a clinical setting and motivate those who smoke by offering evidence-based behavioural support and Brief Intervention Advice.

As well as offering patients advice and support, Practitioners will also be expected to carry out Carbon Monoxide (CO) monitoring and issue appropriate Nicotine Replacement Therapy (NRT).


Post holders will also work closely with community stop smoking services and will encourage patients to engage with and utilise these services once discharged from Hospital to ensure they stay on a smokefree path.


This role is intended to tackle health inequalities across our region by providing an outstanding service by educating, supporting and guiding those who need intervention to stop smoking for good.

Overview of Key Responsibilities

  • Educate and motivate patients to stop smoking by offering motivational and behavioural support.
  • Encouraging patients to stop smoking by educating them around the benefits and informing them of the risks should they continue to smoke.
  • Conduct a comprehensive inpatient smoking assessment including full smoking history, past quit attempts, use of NRT and current motivator(s) to quit.
  • Respond effectively to referrals into the Tobacco Dependency Treatment Service within agreed timescales.
  • Develop personalised treatment plans for patients who are tobacco dependent, in line with the National Centre for Smoking Cessation and Training (NCSCT) Standard Treatment Programme for patients, to support complex behaviour change.
  • Work flexibly across all Merseyside and West Lancashire (MWL) sites as required, for the delivery of the tobacco dependency treatment service.
  • Develop and maintain close working relationships with wardbased medical, nursing, pharmacy staff and communitybased stop smoking services.
  • Conduct and interpret Carbon Monoxide (CO) monitoring to a high standard.
  • Dispense NRT according to the Trust protocol.
  • Facilitate hand over to the community smoking cessation discharge teams on discharge.
  • Follow up patients appropriately following discharge from Hospital according to service protocol and pathways.
The Secretary of State has approved the partnership between St Helens & Knowsley and Southport & Ormskirk Hospitals.

The post holder will:


  • Work with mínimal indirect supervision operating within the assessed level of competence and be accountable for all aspects of own work.
  • Prioritise own workload according to the needs of the service and their individual caseload.
  • Ensure a high degree of confidentiality, always complying with Information Governance policies.
  • Accurately record and report activities and results.
  • Support the delivery of the service key performance and reporting indicators for tobacco dependency.
  • Liaise with others within the Trust to promote the Tobacco Dependence Treatment Service and encourage appropriate referrals.
  • Utilise persuasive, motivational and empathic skills as required because there may be barriers to understanding, alongside limited or fluctuating motivation to change.
