We are currently looking for a Community Care Officer for a role in Nottingham.

The Community Care Officer will work within a Community Learning Disability Team (CLDT), which organises the response to the community care needs of adults with moderate to severe learning disability.

To work within current legislation and the Council's policies, procedures and core values.

Progression to Level 2 is dependant on an assessment of relevant skills and abilities.

Level 2 staff will have at least 2 years experience of working competently in a community care or related service.

Level 2 CCOs are expected to work independently, under the general direction of the Team Manager, and to provide support and advice to colleagues.


Roles and Responsibilities:


  • To ensure the delivery of appropriate services to safeguard vulnerable adults and their families, according to their assessed needs.
  • To undertake all levels of Community Care Assessments in relation to the needs of adults and their carers against the Department's agreed eligibility criteria and in accordance with National Performance Indicators within set timescales
  • To create and implement culturally appropriate care plans and services that demonstrate flexibility and innovative use of resources and undertaking a care coordination role.
  • To undertake case management responsibilities that include proactively managing a caseload (including elements of risk management), the monitoring, coordinating and review of care plans and ongoing service provision.
  • To work independently, under the general direction of the Team Leader, to meet deadlines and manage frequently changing circumstances and conflicting priorities
  • To assist other members of the team in carrying out their work e.g. by providing advice, support and mentoring (as appropriate). The postholder may also be required to convey service users, perform other escort duties, assist on home visits, collect information etc.
  • To maintain and produce professional case records and reports which are structured, clear, accurate, timely and evidence based in accordance with the Department's case recording standards and practice.
