The Adult Disability Service provide support to adults with a learning or physical disability.

Our focus is to ensure we enable people to make positive decisions in their lives, promoting their independence and that they can take part in their local community.

We promote personalisation through our assessment process focusing on the wellbeing of the individual.

Through care planning we strive for increased independence, choice and control by offering personal budgets to enable people to direct their own support.

An exciting opportunity has arisen for an Advanced Practitioner to join the Adult Disability Review Team East.

For this brand new role you will need to be a registered Social Worker or Occupational Therapist and you will have experience of working withinConnected Lives principles in a fast paced environment.

You will review care packages and develop innovative support plans, build networks and raise awareness in the area whilst and develop valuable links for the team.


You will need to be a confident individual, capable of working with the most complex cases, and taking on a supervisory role based on developing skill, experience and expertise.

The role is responsible for ensuring that all statutory health and social careresponsibilities are met and delivered to a high standard including safeguarding, assessment, supervision and care management functions.


You will have extensive knowledge and experience in the field of adult social care, with an emphasis on safeguarding practice and related legislation.

You will have the ability to manage competing service priorities and a sound grasp of current legislation,personalisation agenda and meeting the business objectives of the department.

Demonstrating management potential you will have excellent all round communication and negotiation skills working with the Statutory, Voluntary and Independent sector partners.


You will be an experienced SW(E) registered Social Worker or HCPC registered Occupational Therapist who is actively undertaking a specialist role, e.g AMHP, Best Interest Assessor or Practice Educator.

We are based in Stevenage however we encourage smart working with the benefit of a laptop and smart phone.

You would be required to be office based 40% of the week and you have the option of working at home or at one of our touchdowns across the area.

Safeguarding:
Safeguarding children and adults is of utmost importance to Hertfordshire County Council. The Council requires employees to abide by legislation and best practice to enable Hertfordshire to achieve this. This role has been identified as requiringa Disclosure & Barring Service (DBS) check.

Driving Roles Only:

As part of this role, you will be required to travel to other HCC Sites/Residences either using your own mode of transport or a company car.

In light of this, the ability to drive and/or travel around the County is required for this rolea DVLA check will be required on appointment.


English Fluency:

The ability to converse at ease with members of the public and provide advice in accurate spoken English is essential for the post (for those whose language is a signed language the provision of a sign language interpreter who speaks Englishto the necessary standard of fluency will be required).
