Description

As part of the Primary Care Health & Wellbeing service within the VENN PCN Hull, we are looking to appoint a Community Link Worker to support individuals within a primary care setting to assist patients to connect with, and access, voluntary sector and community-based services, activities and support.

Utilising an asset-based approach, the role will include the identification of social issues, social isolation, housing or debt worries that impact negatively on lifestyle behaviours, for example; smoking and weight management.


The post holder will work collaboratively with both lifestyle and wider statutory and community-based services, working as an integral part of the PCN including GP practices and care homes to ensure the provision of effective and timely advice and support, ultimately providing a holistic approach to resolving issues and improving overall health, wellbeing and quality of life.
