We are looking for a motivated and caring individual, preferably with some community experience, but more importantly someone who is enthusiastic about rehabilitation with older people, providing support to people who have long terms needs and who enjoys working in a team.
The Community Rehabilitation Team is a friendly multidisciplinary team, with staff who work together to facilitate early discharge from hospital or prevent unnecessary admission.
A rehabilitation assistant in a multi-disciplinary team is expected to work with other health and social care professionals to develop the ethos of multi-disciplinary, integrated care and seamless services ensuring that patient care is delivered to the highest possible standard.
As a rehabilitation assistant reporting to a registered health professional, the post holder will implement rehabilitation plans for patients in the community and other settings, observe the patient's condition, providing direct care with a rehabilitation and therapy focus.
Flexible working will be required to meet the needs of the patient group.
The ability to work over 7 days is a requirement of the post. The hours of work will be on a rota covering a shift pattern between the hours of 8.00 a.m. and 6 p.m.

Disclosure and Barring Service:

£18 + Administration cost of £3.85 = £21.85. Deducted at £6.85 from 1st month salary and £5 deducted from salary over following 3 months.
Enhanced Check:
£38 + administration cost of £3.90 = £41.90. Deducted at £11.90 from 1st month salary and £10 deducted from salary over following
