Are you a Nurse looking for your next exciting opportunity?
We are supporting the leading specialty pharmacy in Europe who specialise in the treatment of oncology and rare diseases.

They specialise in conducting clinical trials in the home and support their clients with everything from nursing to critical care, testingand monitoring solutions and data collection.

Our client is looking for ambitious and passionate Nurses to work in conjunction with assigned designated hospitals/centres to deliver patient education and support to patients that are released early following a procedure to be cared for in their own home.


As part of this role you will:

  • Be the on-site expert within partner trusts responsible for the smooth and timely transfer of appropriate patients from the hospital ward to the virtual ward to fulfil the trust and client's contractual agreements and ease the capacity challenges of theNHS.
  • Maintaining exceptional continuation of clinical care, you will be supporting appropriate patients to receive care in their home and expedite their recovery. This includes personally managing patient identification, clinical assessment, compiling care pathways,transitioning care to the team at pace and maintaining clear and accurate communication with all stakeholders.
  • As a proactive homecare advocate you will maintain a live understanding of capacity and available resource, proactively advocate transfer of identified patients in order to maintain 100% occupancy in the virtual ward and maximise bed nights saved for thetrust.
  • This role holder will also seek out opportunities to offer additional support to the Trust and link with commercial colleagues to develop strategies to progress these opportunities.

Skills/ Qualifications:


  • NMC registered and HCPC registered
  • Experience of working as part of a MDT
  • Experience of carrying out multiple face to face patient assessments
  • Experience of influencing senior stakeholders / clinicians to progress new ideas / ways of working
  • Previously undertaking holistic clinical assessment skills, including comprehensive risk assessments, and clinical reasoning skills (ideally as a core aspect of job role)
  • Strong interpersonal skills, establishes rapport quickly, persuasive, motivates people, driven to make things happen, positive approach to work, resilient

Salary:

£29,000- £32,000, plus other benefits such as a company car/ car allowance.
