We are looking for a third caseworker to join our Housing Options Service to deliver The Sustainable Housing Partnership Service (SHPS) in collaboration with Bridges Outcome Partnerships and Norfolk County Council.

The Sustainable Housing Partnerships Service (SHPS) is an outcomes-based programme.

The aim is to prevent and relieve homelessness for single people and childless couples, with low to medium support needs who currently fall below the priority need threshold for assistance by the local authority, but for whom the Local Authority has a Duty to prevent or relieve their homelessness under the Homelessness Reduction Act.

Caseworkers will receive referrals from the Housing Options Team and will work closely with individuals to produce Personal Housing Plans that achieve the desired outcomes of the service of preventing or homelessness.

Working towards KPIs and outcomes set by Bridges, caseworkers will produce a person-centred tailored support package to secure accommodation and provide ongoing support for up to eight months when accommodation is secured, with the aim to ensure the ongoing ability to sustain a tenancy and prevent further homelessness.

£25,409.00-£27,852.00 per year
