Þ **Datblygu pecynnau gwybodaeth ac adnoddau ar gyfer defnyddwyr gwasanaethau a gweithwyr proffesiynol er mwyn cynyddu ymwybyddiaeth o faterion trawsryweddol, yn enwedig ynghylch rhwystrau gofal iechyd**

Þ **Gweithio yn agos gydag ysgolion i gynnig cefnogaeth a gwybodaeth pan fo'r angen**

Þ **Casglu data perthnasol, monitro ac adrodd ar weithgareddau yn unol a gofynion y Gronfa Gymunedol y Loteri Genedlaethol**

Þ **Casglu gwybodaeth am anghenion pobl ifanc i adnabod unrhyw fylchau i ddatblygu rhagor o wasanaethau.**

Þ **Support trans, non-binary and gender diverse young people, by providing information and guidance, to allow them to make decisions based on facts and information on issues that affect them.**

Þ **Develop relationships with healthcare and community providers to improve access to services for trans young people**

Þ **Develop information and resource packs for service users and professionals to increase awareness of transgender issues, particularly around health care barriers**

Þ **Work closely with schools to offer support and information when needed**

Þ **Collect relevant data, monitor and report on activities in accordance with the requirements of the National Lottery Community Fund**

Þ **Gather information about the needs of young people to identify any gaps to develop more services.**

**Job Type**: Part-time
Part-time hours: 22.5 per week

**Salary**: £23,022.63-£25,947.01 per year
