We are looking for an enthusiastic and dynamic person to take up a new role of Physical Wellbeing Practitioner, to join the already established Health Check Liaison Team.


We are looking for one Band 4 Physical Well-being Practitioner for 6 months in Lambeth & until March 31st 2024 in Lewisham.

The Physical Well-being Practitioner will deliver a range of well-being interventions and will co-ordinate activity across community services in Lambeth and Lewisham, training

other members of the multi-disciplinary team and ensuring a varied programme of activity is available to patients with Serious Mental Illness The Practitioner will be supported and supervised by the Clinical Service Lead to deliver the local physical health and well-being strategy.


  • To implement a range of physical activity initiatives, working with all members of the multidisciplinary team to ensure patients are offered a range of opportunities to be active.
  • To ensure a comprehensive range of wellbeing interventions is offered to all service users with Serious Mental Illness
  • To provide advice to both service users and staff regarding healthy lifestyle choices
  • To create links with tertiary providers aligned to health promotion and positive lifestyle behaviours
  • Enable our 'hard to reach' service users to access social engagement


The Trust is dedicated to managing and developing a range of mental health services, working closely with service users and carers, our four partner local authorities, ICBs and NHS hospitals.

Services are primarily focused on people with severe and enduring mental illness.

However, it also promotes good mental health, early intervention to prevent more serious problems and provide support to primary care in dealing with people with less severe problems.

The trust has a strong community focus and our services are provided in a wide range of settings including people's own homes, GP practices, day centres, residential and nursing homes, prisons and hospitals.

About the location:

Our Trust headquarters is located at Denmark Hill less than 5 minutes from the train station (zone 2).

We also provide services and operate across other locations, such as London boroughs of Croydon, Lambeth, Lewisham and Southwark; and substance misuse services for residents of Bexley, Lambeth, Greenwich and Wandsworth.


In partnership with clinical leaders, the post holder will devise and deliver suitable training to staff who are supervising and supporting patients to participate in physical activity to ensure guidelines are met for safe, evidence based practice.


Physical activity is aimed at increasing physical capacity to perform occupational activity, and reduce the risk factors associated with morbidity.

Aims and outcomes are around increased socialisation, and to provide a healthy alternative to sitting on wards.


The post-holder will work closely with the Health Check Liaison Team, Primary Care & CMHTs to ensure patients have been screened appropriately as able to participate in physical activity, and seek consultant assurance if any concerns regarding potential contraindication or risks before engaging individuals who may be in a higher risk category.
