Social Worker - Hospital Team - Hillingdon, London - £36ph

Agency recruitment of social worker required to assist ASC Hospital team as permanent worker will be stepping into a secondment for 6 months_
This will be for 36 hours
Mon Fri for 26 weeks. following a Hybrid model, 2 days at the civic centre and the rest working from home. Workers are expected to complete face to face home visits.

covers Hillingdon Hospital for around 85% of discharges,however there are out of borough hospitals that residents are admitted into ie:
Ealing and Northwick Park. Social workers are not based in the hospital, they only attend to complete assessments_
