SWAGGA Primary Care Network (PCN) is a dynamic collaboration between seventeen practices across South Liverpool. We have a vision to improve patient care, tackle inequalities and provide outstanding support for our member practices.
We plan to make a lasting impact upon the communities we serve and are developing ambitious plans for the future.
As a result, an exciting opportunity has arisen for a Care Co-ordinator to join our rapidly expanding team as part of the SWAGGA Primary Care Network.
Who we're looking for?
We're looking for someone who can work as part of a team to support our practices with this key area of work.
You'll need to be a strong communicator with good administration skills but most of all, we're looking for people with passion and compassion.
As a crucial member of the team, you will help us improve care with a focus on addressing health inequalities and working with practices and other service providers to ensure referral pathways, access, information flow and care coordination takes place across these two key areas of work.
The role.
You will be responsible for the day-to-day management of Cancer Services, making reference to national requirements, data management, reporting to the wider PCN and Practice community and ensuring patients are empowered to navigate the local care system.
Where necessary, you will advise the PCN in relation to how services need to be redesigned to improve access and reduce inequalities.
- Supporting practices to effectively record activity and improve QoF scores
- Understand and advise practices and the PCN in relation to national requirements
- Ensure effective communication and information sharing with patients
- Support practices to develop action plans relating to i) cancer screening ii) identifying patients at risk iii) mobile/pop up clinics

Our combined patient population is approximately 100,000 and we work very closely with our community team and other local Healthcare providers.
The post holder will also be required to travel from practice to practice and to other venues during fulfilment of their duties.
Job description
Cancer Services Care Coordinator
SWAGGA PCN has a team of Care Coordinators working across a range of service areas, including Care Homes and Pharmacy Services.
This post will be focussed on supporting Cancer Care Services across the network, but you will also work with clinical and managerial leads within the PCN and member practices and a wider team.
In addition, key requirements of the role include:
- Collecting and analysing data for Cancer Screening services and member practices.
- Auditing and presenting information as requested by clinical leads.
- Liaising with GPs and other professionals and/or organisations.
- Organising and attending relevant meetings with interested parties.
- Feeding back key data and information to the wider PCN leadership team.
- Contributing to the development of the PCN's wider strategic plans.
- Promoting Cancer Screening to the patient population.
