Description

We are looking for a compassionate practitioner who is self motivated, tenacious, and can use their initiative when working with this vulnerable group of children and young people.


You need to be skilled in communicating with young people and professionals, and adept at keeping data and driving forward excellence in Children Looked After practice.


You will be working with a highly motivated team of professionals who ensure that "no stone is left unturned" to provide for all aspects of health to our cohort.


As an NHS Trust we strongly encourage and support vaccination as this remains the best way to protect yourself, your family, your colleagues and of course patients and service users when working on our healthcare settings.


This post will contribute to improving the health of Camden's Children Looked After through promoting and supporting children and young people to develop healthier behaviours and lifestyles.

It focuses on engaging with individuals and groups, using motivational interviewing and empathic skills, offering them practical support to change their behaviour to achieve their own choices and goals, as well as being a practical resource to help connect foster carers, children and young people (including separated children) into services at a local level.


To enter client data into the relevant computer-based data entry systems, to create reports from the database as necessary and to manage other information systems.

To manage a case load of Children Looked After and Separated Children, enabling groups and individuals (and where appropriate, their foster carers) to change their behaviours and improve their health, using motivational interviewing.


To manage the Health Tracker Database, creating reports and liaising with the health and social care teams to ensure that data is recorded accurately and within timeframes.
