Our mission in Chiltern Hills PCN is to strengthen, develop and support local general practice, to enable the delivery of high-quality care to local people.

We are aiming to build strong clinical teams within and across the PCN.

We are now at the very start of an exciting journey of building a team of additional clinical roles to support and enable the delivery of services within our PCN.

Chiltern Hills PCN comprises 6 GP surgeries in Central Bedfordshire with a combined population of approximately 54,000 patients.

We enjoy excellent collaborative working relationships and we are excited about expanding our team to improve health care services for our patients and to support our clinical and administrative staff.

Our practices are high-achieving and innovative, and have a strong ethos of teaching and training. We will offer mentoring and educational support from our PCN Clinical Pharmacist, GP prescribing and clinical leads.

Main duties of the job Provide first point of contact care for patients presenting with undifferentiated, undiagnosed problems by utilising history-taking, physical examinations and clinical decision-making skills to establish a working diagnosis and management plan in partnership with the patient (and their carers where applicable); Support the management of patients conditions through offering specialised clinics following appropriate training including (but not limited to) family planning, baby checks, COPD, asthma, diabetes, and anticoagulation; Provide health/disease promotion and prevention advice, alongside analysing and actioning diagnostic test results; Develop integrated patient-centred care through appropriate wording with the wider primary care multidisciplinary team and social care networks; Utilise clinical guidelines and promote evidence-based practice and partake in clinical audits, significant event reviews and other research and analysis tasks; Overview of your organisation You will be employed by Chiltern Hills PCN which comprises of Caddington Surgery, Eastgate Surgery, Kingsbury Court Surgery, Priory Gardens Surgery and West Street Surgery.

The role is based over 5 days a week, 4 of which you will be working at Priory Gardens Surgery.

The other day will be a PCN day
