About the Service - Housing & Homelessness Services
A safe home is the solid base from which you build your life.
We support people who are experiencing homelessness, or at risk of becoming homeless, as well as providing specialist accommodation and outreach services for rough sleepers, veterans, survivors of domestic abuse, women and young people.

The Fells is a forty-nine bedded supported accommodation for males, located in Plawsworth, Durham.
The aim of the project is to break the cycle of homelessness and integrate our people who access our service successfully into the community.
An exciting role has arisen for a mental health support worker at The Fells. This is part of Mental Health team to work alongside a passionate and vibrant support staff team.The mental health team will deliver a bespoke mental health service to residents living in the service to ensure improved access to mental health support and advice.
Ensuring necessary interventions are undertaken in a timely manner and ensuring those leaving The Fells are tenancy ready and able to manage their needs in the community.
The service should aim to reduce the need for statutory services and interventions and work alongside other professionals to ensure a partnership approach to meeting service user needs.
The mental health team will work with the service user to identify occupational therapy goals as part of an overall plan.
This will include developing and implementing specialist interventions for individuals or groups, in collaboration with the service user(s) and MDT to achieve therapeutic goals.
