Job summary


Pharmacy Care Coordinator is a key role providing the link between the pharmacy team, patients and practices in the Primary Care Network ensuring seamless medication management.

The role will support member practices in coordinating key activity including access to services, advice, and information, and ensuring health and care planning is proactive, supportive, holistic and patient-centred.

The role will include supporting digital initiatives and includes responsibilities for the co-ordination of the patients journey through primary care and the wider health and care system.


Main duties of the job


You will be supporting the surgeries in Weymouth and Portland in this exciting and developing role that involves working closely with the PCN pharmacy team and the wider multidisciplinary team.

The role will be pivotal in helping to deliver the best possible outcomes for patients, relating to medicines.


You will need outstanding people skills, a caring nature and good IT skills with a desire to learn in this ever-changing environment.

An ethos of promotion of independence, shared decision making, personalisation and partnership-working is integral to this role.

Job description

Job responsibilities

  • Work with individual practice teams and primary care professionals within the PCN to identify and manage a caseload of patients
  • Work closely and in partnership with PCN colleagues predominantly Clinical Pharmacist(s) and Pharmacy Technicians but may also include the wider PCN multidisciplinary teams social prescribing link workers and health coaches
  • Signpost team members, patients and carers to relevant services as appropriate
  • To work and act accordingly to protocols and SOPs to support medicines optimisation
  • Work with the pharmacy team to proactively identify people who could benefit from their support, utilising the resources available to them. E.g. Ardens searches, building new searches
  • Make referrals to the multidisciplinary team as appropriate
  • Contribute to the development of personalised care and support plans to best meet their needed when this is appropriate Increasing patients understanding of how to manage and improve health and minimise harm from their medication.
  • Actively support and uphold excellent medicines optimisation within the PCN
  • Regularly liaise with the pharmacy professionals in the PCN who are involved in the persons care, facilitating a coordinated approach, and ensuring everyone is kept up to date so that any issues or concerns can be appropriately addressed and supported.
  • Develop an indepth knowledge of the local health and care infrastructure, with expertise in the pharmacy team and know how and when to enable people to access support and services that are right for them.
  • Support patient independence, promote healthcare and wellbeing including the use of digital apps to support their care
  • Supporting patients and carers to utilise decision aids when appropriate
  • Support patients to have shared decisionmaking conversations
  • Facilitate smooth and planned discharge and handover between care settings across the health and social care system, including GP, acute, community, and be responsible for facilitating interagency communication and support with a focus on medicines elements
  • Use healthcare technologies to optimise service delivery, access, and continuity of care for patients and carers
  • Use Population Health Intelligence and Risk Stratification tools proactively to identify groups of patients for MDT discussions or to deliver health services e.g.structured medication reviews
  • May attend and present at MDT meetings
  • May support the coordination and delivery of surgery medicines and pharmacyfocused meetings

Person Specification

Qualifications

Essential

  • Good general secondary education including Maths and English

Desirable

  • Presentation skills

Experience

Essential

  • Data analysis and interpretation
  • Experience of working with patients or the public in a customer care environment.

Desirable

  • Primary Care experience
  • Working in a healthcare setting as part of a team, especially a pharmacy environment
