In line with this vision, CNWL have created multidisciplinary, health and social care Community Mental Health Hubs which are aligned to, and will work closely with, borough Primary Care Networks.

This post holder will be a member of the South Kensington and Chelsea Community Mental Health hub delivering care to a set population of the borough.

The post holder will be a member of the multi-disciplinary and carry a caseload with an intervention focus. You will provide a range of evidence-based interventions to help promote recovery.

The Team serves the population of South Kensington & Chelsea, a diverse and vibrant borough. It is an established team with a friendly, supportive culture. There is a visible, supportive management team and a strong emphasis on training and professional development opportunities.

Key Responsibilities

Clinical

1.1 To provide management of a caseload of service users with enduring and complex mental health needs through provision of therapeutic care and treatment in the individual's own home and other community venues.

1.2 To assess health and social care needs and negotiate care plans with service users and their carers, remaining objective when working with diagnostic uncertainty.

1.3 To undertake risk assessments, according to Trust policy.
1.4 To proactively undertake and ensure rigorous risk assessment & risk management strategies are applied. To ensure maximum 'risk tolerance' facilitates the individual's recovery, in line with risk policies and local guidelines.

1.6 To plan, implement and review individually tailored programmes of intervention

1.7 To support service users and colleagues to ensure that care plans are developed in partnership with the individual and that they reflect 'recovery goals'.

These should be strengths-based and oriented towards reinforcing a positive identity and developing valued social roles.
1.8 To work in partnership with other agencies and offer supervision and guidance to external agencies as required.

1.9 To identify and implement relevant procedures in relation to vulnerable adults, safeguarding, child protection, capacity and Deprivation of Liberty (DoLs) issues.

1.10 To identify carers needs in line with national requirements.

1.11 To support service users in relation to their physical health, employment, and housing and welfare benefits issues; referring to specialist provider services as necessary.

1.12 To provide all service users on caseload with a full adult mental health assessment and offer regular monitoring of mental state and medication monitoring.

1.13 As a Key Worker, to co-ordinate and manage all aspects of an individual's treatment and care; this will include retaining an overview of work by individuals from partner agencies.

1.14 To participate in the hub's triage function and to receive referrals from GP's and other sources
1.15 To participate in the hub's duty rota.


  • Communication

2.1 To demonstrate excellent communication skills and form positive therapeutic relationships using warmth and empathy and enable service user empowerment.

2.2 To establish and maintain effective communication networks with service users, with complex learning, emotional and behavioural needs, their carers, other professionals and agencies in order to ensure a comprehensive and consistent approach to client care.
