Support Team Member - Recall & Research (Full Time - 37 hrs per week)

We are looking for an enthusiastic new colleague to join our Support Team.

Focusing on our Recall and Research processes, you will run a programme of work to invite, track and ensure that all of our patients with long-term conditions receive their annual reviews, ensuring that targets are met and we keep on schedule.

You will also run a number of processes to help us monitor patients on certain treatments and who may need future hospital follow-ups.


You will also provide admin support to our Research Lead, helping to run our research activity and maintaining contact with the study leads and working with our practice teams to help deliver the studies.

You must be able to demonstrate excellent organisational and communication skills and be able to prioritise your workload.

Recall & Research
-
Audit - Provide admin support to the Audit lead and undertake patient and data audits as requested. Support analysis of data and agreed actions with audit lead/audit initiator and track through. Enter and record audits on TeamNet. Contribute to audit summary/review/discussion at Practice Meeting.

Research admin - Provide admin support to the Research Lead to ensure the timely and accurate completion of any research projects undertaken by the practice. As a ppoint of contact with the appropriate research commissioners and action and process data requests and submissions relating to each study. Provide regular research updates as requested by the practice.

LTC recall and capacity reconciliation -Run the practice long-term-condition recall process and recall appropriate patients on a monthly basis. Undertaken an available capacity review with the Lead Nurse before invites are sent and liaise with Reception to advise when recalls have been sent. Proactively recall and book any vulnerable/hard to reach patients to assist completion of their review. Recall any other patient groups for clinical activity as determined by the practice. Ensure that appropriate read codes are added to patient records to record their recall status and exception code where appropriate. Provide regular updates to the Recall Lead.

S1 Clinical Searches - Undertake clinical searches on S1 as requested by the practice and provide support to any colleagues who require a search to provide the data they need.

Workflow/Data Support - Provide workflow/data support as per current protocols to ensure that incoming clinical letters are processed in a timely and accurate manner and that required actions are identified and progressed.

Summarising - Summarising of patient records to ensure that all relevant information is captured and recorded for patients joining the practice.


Patient Data

  • Provide effective and timely scanning service uploading and recording patient data on to the Practice clinical system including identification and coding of new problems, diagnoses and treatments from outpatient, community and discharge letters/reports received by the practice.
  • Adhere to the practice protocol for the processing of incoming discharge, outpatient clinic and other provider (WCF/OOH/111 etc) letters. Ensure that letters are processed in an accurate and timely manner.
  • Provide an effective coding service to ensure that clinical records are kept up to date and that full data is extracted and recorded in a timely manner to support clinical coding, chronic disease register management, Practice QOF achievement, Enhanced Services data capture and financial claims, CCG clinical priorities and any other adhoc or inhouse Practice coding requirements.
  • Action workload in the "for coding" task box.
  • Manage workflow and allocate documents to GPs as appropriate.
  • To provide admin support to all patient recall processes. This could include send bulk letters and/or text messages to involute patients for reviews, clinics or other procedures.
  • Summarise new patient notes in an accurate and timely manner to ensure practice clinical records are kept up to date and that full data is extracted and recorded to support clinical coding, chronic disease register management, Practice QOF achievement, Enhanced Services data capture and financial claims, CCG and LCG clinical priorities and any other adhoc or inhouse coding requirements. Highlight any data issues or concerns with the Data & Systems Manager.
  • Check new GP2GP registrations to ensure that received patient data has been coded in an appropriate way.
