  • To work alongside people with a wide range of disabilities including, physical, learning and mental health disabilities, supporting therapeutic intervention to maximise functional ability, to enable the person to achieve optimum control of their lives,reduce dependency and increase independence. The post holder will be responsible for undertaking specialist functional assessments in order to establish if people meet criteria for the provision of services.
  • To undertake: environmental ergonomic assessment of the person's environment; assessment of cognitive and functional ability to carry out daily living tasks; assessment of moving and handling situations.
  • To work alongside people, carers and staff providing a planned programme of interventions which maximises functional ability with the desired outcome of reducing dependency and promoting independence.
  • To be responsible for providing specialist occupational therapy support to staff, people, formal and informal carers. To participate in project work and to be responsible for a range of liaison functions.
  • To make individual home visits to assess people's needs in accordance with current legislation and antidiscriminatory practice, promoting a strengths based approach and ensuring demonstrable personcentred outcomes.
  • To take and act upon new referrals.
  • To undertake specialist moving and handling risk assessments, advising staff, clients and carers on techniques, equipment and methods of reducing risk.
  • To discuss with people the outcome focussed care plan from their assessment, identifying the choices and options to meet their needs and to maximise their independence, ensuring at all times that value for money is achieved.
  • To be aware of, and to act on identified issues of Adult and Child Safeguarding in line with directorate policies and procedures and to seek support where appropriate.
  • To plan an agreed programme of Occupational Therapy intervention with agreed goals and to ensure that the plan is carried through.
  • To work in conjunction with people, their carers, other professionals and agencies, to coordinate and implement agreed service provision.
  • To provide equipment to assist with daily living and to be proficient in the instruction and demonstration of the equipment to people and carers to enable them to achieve maximum functional independence.
  • To organise any necessary property adaptations with the appropriate involvement of other agencies and to recommend, design and monitor the process for the provision of adaptations.
