Description
Duties And Responsibilities Undertake and record the following tasks: Clinical Procedures (not comprehensive): Smoking cessation support NHS Health Checks Weight management referrals Blood pressure checks Signposting for vaccinations and immunisations Signposting to cancer and non-cancer screening to help increase uptake Signposting to mental health and wellbeing support Any additional tasks/health promotion campaigns as deemed necessary

Administrative Procedures:

Follow infection control guidelines Maintain own clinical notes Maintain confidentiality Undertake other administrative duties where required Link up with PCNs and work with VCSE partners, social prescribers, to provide appropriate support for patients who may find it difficult to access primary care services Promote the work of reducing health inequalities Enable a two-way dialogue, feeding back directly from communities to GP surgeries to improve understanding of the barriers they may face in accessing GP services.

Working closely with the Local Public Health team to identify groups who may experience health inequalities and maximise referrals into preventative services and support.
