Description
We are seeking to recruit a highly motivated enthusiastic and forward thinking practitioner.The post holder will have have opportunity to build upon their existing clinical experience in a supportive environment offering ongoing clinical and professional development.
The post holder will work alongside individuals, their carers, support staff, Social Care, GMP, GMSST and other key stakeholders to ensure best person centred outcomes are achieved.
The post holder will work autonomously and collaboratively within the skilled mixed integrated tea, completing highly specialist assessments whilst developing, implementing and reviewing specialist plans.
