Are you a Registered Nurse or an Allied Health Practitioner (AHP) with an interest in utilising your clinical skills to support a teaching/training role in care homes whilst working as an autonomous practitioner? If the answer is yes, then we have the ideal post for you.


The Care Home Support Team (CHST) in Richmond have an exciting new opportunity for you to join our dynamic team as a clinical digital trainer.

You will be providing training to support embedding remote monitoring to care home staff.

The aim is to reduce avoidable London Ambulance Service (LAS) call outs and hospital admissions whilst working in partnership with the South West London (SWL) Integrated Care Board (ICB) and Primary Care Networks (PCN's).

You will identify barriers, seek to understand the challenges care homes present and identify solutions.


Support care homes with changing clinical practice to integrate core Bluetooth enabled remote monitoring equipment and integrate additional Bluetooth enabled glucometers, spirometers, and stethoscopes, ECG monitors and weighing scales.

To support the increase of care homes signing up to the remote monitoring project

Provide support to care home staff in achieving appropriate clinical competencies

To embed remote monitoring, assessment escalation and communication tools into practice

To have an excellent understanding of the Restore 2 process to support teaching

Increase digital inclusion and mitigate against digital exclusion and inequality by addressing barriers in the care homes such as access to digital equipment, skills and confidence using digital equipment and a lack of motivation to change current processes and day to day clinical activities
