About the Role:


We have an exciting, new opportunity for a care co-ordinator to join The Dengie and South Woodham Ferrers Primary Care Network, to support our GP practices across the local area and to assist in re-shaping primary care across our communities on a full-time basis.

The role involves working very closely with the practices and the multidisciplinary team (MDT) within the PCN. The role is pivotal in ensuring all patients receive the best possible care and service.


The care coordinator will:
- support the clinical directors in coordinating all key activity, including access to services, advice, and information, as well as ensuring health and care planning is timely, efficient, and patient-centred
- support digital initiatives and take responsibility for the co-ordination of the patient's journey through primary care.


If you are a skilled care co-ordinator, with excellent person-centred planning experience and strong local knowledge for accessing key support, then we would love to hear from you.

The ideal applicant will have the following skills, experience and qualifications:

  • Experience of working within healthcare, the voluntary or community sector, to support vulnerable groups;
  • Ideally you will have a Diploma Level 2 in Health and Social Care, or an equivalent qualification;
  • Strong listening and communication skills;
  • Familiar with local resources and services, including how to access them;
  • Motivated by helping people, with care and empathy;
  • Able to deliver personcentred support in a nonjudgmental way;
  • Experience of completing holistic personcentred care planning assessments.

The Dengie and South Woodham Ferrers Primary Care Network is a collaboration of seven GP practices serving approximately 45,000 patients from the Dengie, Bicknacre, Burnham-on-Crouch and South Woodham Ferrers area.
