Responsibilities:
- Monitor and evaluate client's physical, psychological, and social wellbeing.
- Provide protection for the client and their property.
- Manage cases to achieve desired outcomes.
- Record client's information such as medical conditions, history, medications, and treatment plans using our digital care planning system.
- Meet with clients to discuss progress toward their treatment goals.
- Provide services to clients with a variety of medical conditions.
- Work well with teams
