JOB DESCRIPTION

Job Title Wellbeing Team Care Co-ordinator


Responsible To Wellbeing Team Lead


Location Barnstaple Alliance PCN wide.
This position will mainly be based at Queens House
EX32 8HJ but may be required to work from
Brannam Medical Centre, Fremington Medical
Centre, Litchdon Medical Centre and Queens
Medical Centre.


Salary £ £11.00 per hour depending on experience


Hours 25 hours per week


Overall Purpose
To act as the single point of administrative and
coordination support for the Wellbeing Team within the Barnstaple Alliance Primary Care Network. The
Wellbeing Team currently consists of the Team Lead,
4 Wellbeing Link Workers and 2 Wellbeing
Connectors.

This administrative role will be pivotal in ensuring all patients receive the best possible care and service through coordinating and supporting the delivery of all key activities of the Wellbeing Team.

This includes access to services, advice and information, ensuring a patient centred approach with timely and effective care planning and organisation.

The role will involve co-ordination of referrals to the service from the member practices to the team, and the patient's pathway.


MAIN DUTIES & RESPONSIBILITIES
Key responsibilities for the position will include but are not limited to the following:

  • To be the single point of access for the Wellbeing Team for practices and other professionals, as appropriate.
  • To be the single point of access for administration and coordination support for the Wellbeing Team.
  • To work closely with the Wellbeing Team Lead with the day to day activities of the Wellbeing Team and service.
  • Receive and acknowledge new referrals and contact the patients to keep them informed.
  • To explain and promote the wellbeing service, by providing links and supporting with lowerlevel signposting to service users.
  • To oversee the referral inbox to ensure time sensitive tasks are directed and responded to in a timely manner.
  • To support the Wellbeing Team by monitoring team inboxes, including but not limited to TPP SystmOne and other record keeping/social prescribing software.
  • To offer administration support to new and pilot projects that include the Wellbeing Team or service.
  • To support patients with requests for certain community services and support funds.
  • To liaise with the Primary Care Flow Team including the Flow
  • Coordinator to arrange meetings and work collaboratively as needed.
  • To provide support to Wellbeing Team members attending the Mental Health Multi-Agency Team (MAT) and other Multi-Disciplinary Team (MDT) Meetings as appropriate.
  • To develop and maintain a directory of services for the team and the wider PCN.
  • To keep practices and patients informed of latest developments and resources as appropriate.
  • To work with the Wellbeing Team Lead to plan and coordinate Wellbeing Team activities such as holidays, sickness, celebration events, Annual General Meetings (AGM) and other meetings.
