    Key Health is seeking an Experienced Social Worker to join a dynamic Adults Community Team in Birkenhead. This short-term position offers a unique opportunity for a skilled professional to deliver high-quality social work that safeguards and empowers adults and families.

    What You'll Do:

    • Independently manage a diverse and complex caseload.
    • Complete Care Act assessments, support planning, and mental capacity assessments.
    • Handle safeguarding concerns, risk assessments, and reviews.
    • Collaborate across departments and external agencies for integrated care.
    • Offer expert advice and represent the team in professional settings.
    • Maintain accurate records and contribute to court reports where required.

    Key Responsibilities:

    • Provide timely information, advice, and access to services to reduce risks and improve outcomes.
    • Conduct statutory assessments and create person-centred support plans.
    • Participate in multi-agency working and case discussions.
    • Ensure legal and ethical compliance with HCPC standards.
    • Support colleagues with professional insights and direction.
    • Demonstrate best practices and contribute to service improvement.

    Requirements:

    • Must be eligible to work in the UK (No sponsorship available).
    • Minimum 2 years UK experience in a social work role.
    • Background in Local Authority Adult or Children's Services.
    • HCPC registration (must be valid and evidenced).
    • Recognised Social Work qualification (e.g., Degree in Social Work, DipSW).
    • Full UK driving licence and access to a vehicle.
