A rewarding opportunity has arisen for a Senior Community Practitioner within the Intensive Life Skills Pathway Team based in Shropshire to help those in need with your caring and compassionate support.

We are a specialist community team providing assessment, formulation and intervention using a Structured Clinical Management (SCM) framework.

Our focus is on treatment, recovery and enablement to increase peoples emotional social and practical functioning, through a combination of group skill based learning and individual therapy.


We are looking for any current Nurses, Social Workers & Occupational Therapists who are looking to be part of this exciting role where you will have the opportunity to access further development and training pathways within this role.


  • As part of this role, you will be working in a specialist adult community mental health pathway and responsible for an identified caseload of service users.
  • You will be responsible for undertaking clinical assessments in accordance with the clinical pathway, to establish the person's needs, goals, risks, and strengths, to inform and generate a formulationbased care plan agreed with all involved.
  • Responsibility for delivering evidenced based therapeutic interventions on a 1:1 and group basis, in accordance with an agreed care plan.


You will also have the opportunity to assist with consultations, support and clinical supervision to other health professionals working within the Community Mental Health Service and other areas of the Trust, where identified as appropriate.


  • A relevant nursing qualification or other registration
  • RMN / Occupational Therapist / Social Worker / Therapist.
  • Post qualification experience, such as evidence of formal courses or training relevant to working with people with complex mental health needs, related to emotional instability.
  • Ability to deliver group and individual interventions.
  • Ability to work with service users expressing high levels of distress.
  • A positive attitude and a caring and compassionate approach.
  • Good communication skills
