  • Are you passionate about improving the health and wellbeing outcomes for people in Calderdale? Would you be interested in expanding and developing your career in Adult Social care? If so, you could be the person we are looking for.
  • Calderdale strives to work in an integrated way, particularly with our local health services. Within Calderdale, the Hospital Discharge Team, as part of the Early Help and Prevention service, support in the discharge planning for the people admitted to Calderdale Royal Hospital and Huddersfield Royal Infirmary. The team is made up of Local Authority social workers, service coordinators and provision planners but also works very closely with health colleagues within the NHS. The role of the provision planners is very new, however has been successful in supporting social workers, service coordinators and health colleagues with discharges from hospital so this is being expanded further.
  • We are seeking individuals who are passionate about adult social care and strive to work towards a more integrated model of working between health and social care. You will want to provide a highquality service and are keen to develop your professional experience and knowledge. You should be able to work independently but also part of a team that is made up of health and social care colleagues. You should be organised, able to work under pressure in a busy team and have enthusiasm to develop and change multiagency working.
