This role will involve shifts between 07:30 and 20:00 and weekend working.

The main duties of a Patient Pathway Coordinator include:
Operational

Managing clinical visit allocations
Rota management
Receive patient referrals and action in line with departmental requirements
Coordinate and provide administrative support to relevant teams
Action daytoday issues, proactively finding resolutions and escalating issues to management

Communication

Communicate with multidisciplinary team effectively, developing strong working relationships with clinical and managerial staff
Communicate with patients to resolve queries and ensure excellent patient care
A key link between social and therapy teams to support efficient and safe discharges

Policy Development

Review and support development of Standard Operating Procedures and internal targets

Data Collection and Analysis

Monitor and review daily clinical capacity of Rehab Support Workers
Collecting and updating patient information in line with local policy and maintain accuracy of data at all times

Staff Management

Supporting staff training needs
2 hour, same day or next day response to support early dischargefrom hospitals from Guy's & St Thomas, King's College and is integrated with secondary care.
Hospital admission avoidance, with 2 hour or same day response, working in conjunction with London Ambulance Service (LAS), GPs, Community Matrons, District Nurses, Social Care and the other community therapy teams.
