This role will involve shifts between 07:30 and 20:00 and weekend working.


The main duties of a Patient Pathway Coordinator include:
Operational

  • Managing clinical visit allocations
  • Rota management
  • Receive patient referrals and action in line with departmental requirements
  • Coordinate and provide administrative support to relevant teams
  • Action daytoday issues, proactively finding resolutions and escalating issues to management
Communication

  • Communicate with multidisciplinary team effectively, developing strong working relationships with clinical and managerial staff
  • Communicate with patients to resolve queries and ensure excellent patient care
  • A key link between social and therapy teams to support efficient and safe discharges
Policy Development

  • Review and support development of Standard Operating Procedures and internal targets
Data Collection and Analysis

  • Monitor and review daily clinical capacity of Rehab Support Workers
  • Collecting and updating patient information in line with local policy and maintain accuracy of data at all times
Staff Management

  • Supporting staff training needs
  • 2 hour, same day or next day response to support early dischargefrom hospitals from Guy's & St Thomas, King's College and is integrated with secondary care.
  • Hospital admission avoidance, with 2 hour or same day response, working in conjunction with London Ambulance Service (LAS), GPs, Community Matrons, District Nurses, Social Care and the other community therapy teams.
