Description

To work in the Primary Care Team, supporting practices within Portsmouth as part of Hampshire and Isle of Wight Integrated Care Board (ICB).

The team has the dual purpose of ensuring that practices are effective and active members of the ICB through the provision of information, effective communication and the management of processes and of the day to day commissioning of General Practice whilst leading and supporting practices through the transformation agenda.

The post holder reports to the Primary Care Improvement Manager and Head of Primary Care Commissioning, leading specific projects within the team as required.


The post will act as an initial point of contact for practices, offering support and solutions where practicable or sign-posting as appropriate.
